Please call in advance to confirm the offices will be open. Each physician may follow a different schedule. (For example, a certain physician may be available to speak over the phone and for appointments only on Wednesday's and Friday's.)
Should you have any questions or need to reschedule your appointment please call our office at 817-375-5200. We will be happy to assist you. Due to heavy call volume during the early morning and late afternoon hours, we ask that you call between 11:00am and 3:00pm all possible.
Emergency calls will be handled immediately and calls which are less urgent may be returned at a time when the physician is less occupied. Outside of regular business hours our telephone is answered by an answering service and messages will be transmitted to the doctor.
New Patients
Patients that are new to our practice will need to complete medical information forms. The forms can be emailed, faxed or brought with you to our offices.
