Thank you for selecting Arlington Orthopedic Associates for you healthcare needs. We are committed to maintaining the highest standards in orthopedic care and providing the best possible service to our patients.

To better serve you, we ask that you bring all the following items to your appointment:

  • Completed paper work (Enclosed)
  • Referral from your Primary Care Physician (If required by your insurance company. Most HMO's require a referral.)
  • Any recent X-ray or MRI films related to your condition
  • Any recent tests related to your condition
  • List of all medications you are currently taking
  • Insurance card
  • Payment (We accept MasterCard, Visa and Care Credit)

Should you have any questions or need to reschedule your appointment please call our office at 817-375-5200. We will be happy to assist you. Due to heavy call volume during the early morning and late afternoon hours, we ask that you call between 11:00am and 3:00pm all possible.

Each new patient will be asked to complete 4 different forms: (1) Patient Information Form, (2) Review of Systems Form and (3) Eligibility Form and (4) General Orthopedic Form specific to condition: i.e. Knee Form, Shoulder Form, etc.

  3. Eligibility Guarantee Form
  4. Please also complete one of the following forms:
    General Form
    Knee Form
    Shoulder Form

If you want to submit the form by FAX follow these instructions:

  1. Open the appropriate form. (The form will open in a PDF format)
  2. Choose Print (which will print a hard copy) for you.
  3. Once all the needed information has been filled in, fax the form to our office:
