How to Request Your Medical Records

In partnership with Sharecare you can now request your medical records online. Your online request goes directly to Sharecare where processing begins within 48 hours. If there is a charge then Sharecare will contact you directly.

If you are unable to complete the electronic form above, download and complete the paper form below. We will be happy to submit your request to Sharecare on your behalf. The authorization form must be legible and complete. Authorizations that are illegible or incomplete will be returned.  Please note the processing of records is faster using the online form above.

Sharecare records are usually available within 10 days from from online form submission. However, if you are picking up your medical records in person, please allow 15 business days for completion.  Be sure to bring a government-issued ID when picking up your records in person. To arrange for another individual to pick up the documents for you, please indicate that on the authorization form, and ask them to bring their government-issued ID.

Only the patient, parent/legal guardian, or the patient’s legal health care representative can sign the form to release medical records. If you are requesting records on behalf of the patient or as the patient’s representative, please provide a copy of an Advance Directive/Durable Power of Attorney for healthcare/Conservatorship. 

 

Online Requests for Disability/FMLA

Once you have submitted your form, Sharecare will contact you within 48 hours to collect payment for processing. We are happy to accept and submit the forms on the patient’s behalf however for more prompt and complete service we suggest online submit directly through Sharecare.

Requests from Third Parties

If you are not a patient and need to submit a request for the medical records of an AOA Orthopedic Specialists patient, you may submit your request along with the patient’s authorization here.

After you have submitted the form with the patient’s authorization, Sharecare will contact you to move forward if payment is required prior to processing. Otherwise, Sharecare will promptly process your request.
